LOW BACK PAIN...ANSWERS
 
 
 
Most of my adult life I have had some kind of back pain. I do have arthritis, osteoporosis and some degenerated discs in my back. For several years, medication, physical therapy and chiropractic adjustments have helped. About 2 1/2 year ago I took a bad fall, injured and bruised my left buttocks and side area badly. After some time, what was thought to be just a bad bruise continued to still cause a lot of pain and affected my walking. I was told I had Spinal Stenosis. I chose to have the epidural shot that for eleven months was great. Then the pain started again and I had a hard time walking... I came in (The Morris Spinal Stenosis and Disc Center) for probably one of the best complete examinations I have ever had for my back and body structure. I chose to do the treatments... I am feeling so much better now. I will probably always have some back pain because of my history, but the stenosis is no longer bothering me...I am not taking any pain medication... I feel so fortunate that I discovered Dr. Morris. I have referred them to others and I am so thankful for the help that they have given me.

Let's first look at the CAUSES of low back pain (see the column to the right).
 
  • The facets (the parts of the spine that connect to one another) "jam" into each other. Patients often find their back pain is relieved by bringing their knees to their chest and is worsened by prolonged standing or wearing high heeled shoes.
  • The discs can herniate into the spinal nerves or spinal canal and causes severe, sharp pain to the back or leg. Bending forward and sitting increases the pain.
  • About 5-10% of us have uneven leg lengths causing the spine to wear unevenly. It is nearly always misdiagnosed and leads to chronic pain and "instability."
  • Misalignment of the sacroiliac or lumbar vertebrae is a common cause of low back pain that is usually mistreated by the medical community.
 
 
Of course these aren't the only causes of back pain, just the more common ones we see in our clinic.

Sources of Pain
 
Common sources of low back pain include the disc, the spinal nerve and the facets.
 
 
 
Disc Herniation and Degeneration
 
This MRI shows a side view of the lower back. Notice how the disc "bulges" into the spinal canal. The lower disc is narrowed due to degeneration (arthritis). Sacroiliac Irritation and Degeneration
 
Notice how the Sacroiliac Joint is whiter on the right side. This can be a sign of chronic stress and degeneration and is an often missed cause of lower back pain.
 
 
 
Sacroiliac Ligament Sprain or Laxity
 
The sacroiliac ligaments can become lax or weak just like the ligaments in an ankle that's been sprained too many times. This is not visible upon X-ray or MRI and is often mistreated and misdiagnosed.
 
 
Lower Back Muscles
 
Lower back muscle imbalances due to weakness, old injuries, bad posture, etc. must be properly evaluated and treated or a history of chronic low back pain will ensue. Unfortunately,
X-rays and MRIs are not helpful here. It depends on the skill of the doctor.

 
 
Some Back Problems Require Surgery
 
Notice how L5 fractured and slid forward on S1. Surgery is rarely necessary. In those cases we work closely with the surgeon to develop the best rehabilitation program.
